Ahwatukee Family Medical Center participates with Medicare and many commercial insurance plans, but due to multiple changes in the health care industry, we are unable to provide a complete list of contracted insurance companies. It is your responsibility to contact your insurance company to verify the Dr. James Nichols is contracted and in-network with your insurance plan.

Insurance policies vary and it is important to know the details of your plan.

If you have questions about your individual plan, please contact your plan directly.

It is the patient's responsibility to be informed about their insurance coverage, including (but not limited to) benefits, deductibles, referrals, authorizations, co-payments, coverage/coverage limitations, patient responsibility, etc.

It is the patient's responsibility to make payment arrangements for services rendered by other third party facilities for things like laboratory services, radiology, diagnostic services, etc.

It is important that you bring your insurance card to every appointment. If you change insurance companies or plans, or change your address, phone number, email, etc., please contact our office as soon as possible so we can make the necessary changes to your file.
